Comprehensive, Coordinated Healthcare
Once operational participants of Piedmont
Health SeniorCare will receive an individualized plan of care managed
on a daily basis by an interdisciplinary team of providers.
This team will include
geriatrics-trained physicians, nurse practitioners & physician
assistants, nurses, social workers, nutritionists, rehabilitative
therapists, transportation personnel, in home aides, and additional
providers as needed.

Access to care will be available 24 hours
a day, 365 days a year.
The majority of services will be provided
at Piedmont Health SeniorCare's Day Center. Some
services will be provided in-home. Additionally,
Piedmont Health SeniorCare will contract with existing community
providers for services we are unable to provide ourselves, including
specialty services and inpatient hospitalization. Through such
collaboration, we will meet all of our participants' needs and provide
the highest quality of care.
